Based on the observation and record review, the Licensee did not meet the above regulation which poses an immediate health, safety and personal rights risk to the children in care. During the facility tour the LPA observed S1 and S2 providing Care & Supervision to seven infants. After reviewing the staff files, the LPA was unable to review complete transcripts with required classes for S1or S2. S1 did not have any transcripts and S2 did not have Infant/Toddler credits.

During the facility tour the staff acknowledged C4 can roll from their stomach to back on their own. During children’s file review the LPA was unable to review an LIC 9227 with Section C completed.
