Based on interviews and self report conducted, an absence of supervision occurred when a child was left alone outside. This posed an immediate health/safety and personal rights risk to children in care. This is a repeat violation. 8 An immediate civil penalty of $1000.00 was assessed on today's visit. An informal meeting may be scheduled by the Department at the discretion of the Licensing Program Manager (LPM).

Based on record review and interview, the licensee did not comply with the section cited above in that Staff 2 (S2) and Staff 5 (S5) did not complete renewal of madated reporter training every 2 years. S2's mandated reporter training needed to be renewed by 8/27/2025 and S5's mandated reporter training needed to be renewed by 10/20/2025. This poses/posed a potential health, safety or personal rights risk to persons in care.
During today's visit, preschool playground was inspected and found to be in substantial compliance. Director agrees to submit updated facility sketch to reflect all new playground changes and statement of estimate renovation completion. Update progress via LIC 624 to CCL. Per Director, Alisa Holtegarrd, center playground renovation/construction began 09/07/23; this will continue throughout the other playgrounds at the center one at a time until all play areas are completed. CCL was not informed of construction changes being done. "This poses an potential Health and Safety risk for the children in care".
Based on observation and interviews, LPA's observed two children using the bathrooms unattended. The door to the bathrooms were closed when the children were inside the bathrooms, each child entering the bathroom by themselves, while the teacher stayed outside in the classroom. which posed a potential Health, Safety, and Personal Rights risk to the subject child and potential children in care. 8 Director stated a work order was submitted to remove the full doors in the preschool bathroom and install half doors. In the meantime, Director stated they will be making alterations to the existing door to make them into half doors. Director agrees to submit pictures to CCL once completed.
Based on observation, the licensee did not comply with the section cited above in 6 restrooms floors and 6 toilets observed were unclean, which poses a potential health, safety or personal rights risk to persons in care.
Based on interview, record review, the licensee did not comply with the section cited above in by admission stated the facility kitchen outlets and water fountains used for water consumption and food preparation have not been tested for possible lead exposure, which poses a potential health, safety or personal rights risk to persons in care.

Based on Interviews and records reviewed, on 08/04/22 Staff were transitioning between scheduled breaks, in this timeframe a child was struck multiple times and kicked by another child and as a result, sustained a fractured ankle before staff intervention. This poses an immediate health and safety risk to the children in care. 8 visual supervision and transitioning between scheduled breaks. Documentation of training/proof of correction will need to be submitted to LPA on or before the date of correction, 09/13/2022. Office meeting with CCL Management will be scheduled at a later date.
Based on interviews conducted with staff on 08/05/2022, LPA discovered the facility had knowledge of an unusual incident which occurred on 08/04/22 regarding a child sustaining an injury while in care and did not report it to licensing. This poses a potential risk to the health and safety of children in care. 8 proper time frame they must be reported to licensing. Documentation of training/proof of correction will need to be submitted to LPA on or before the date of correction, 09/26/2022
