Personal Rights (a) The licensee shall ensure that each child is accorded the following personal rights: (2) To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs. This requirement was not met as Licensee Representative will add a staff member into the Discovery Preschool classroom as of 03/06/2026. The 3 yr olds will be transitioned to a Preschool 3 yr old classroom on 03/16/2026. Licensee Rep will email LPA an updated LIC 500 Personnel Report for both staffing evidenced by staff not preventing children in care from engaging in harmful behaviors that resulted in injuries. 8 changes by 5:00 PM on 03/06/2026.
LPA observing accessible cleaners 1 Licensee will include hazardous items training in her staff meeting on 03/10/2026. Licensee will email LPA training signatures by 5:00 PM on 03/19/2026. and unlocked child locks in the Discovery Preschool classroom on 03/05/2026.
Based on LPA observation, the licensee did not comply with the section cited above when the preschool playground has an uneven surface with cracks and a hard surface under the play structures which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on observation and record review, the Director did not ensure a criminal clearance was obtained for 1 staff member which poses an immediate health, safety and or personal rights risk to chidlren in care.
