To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse or other actions of a punitive nature including but not limited to: interference with functions of daily living including eating, sleeping or toileting; or withholding of shelter, clothing, medication or aids to physical functioning. 1 Facility placed staff immediately on suspension pending facility investigation. Deficiency Cleared. This requirement was not met by a staff was observed grabbing a child by the arms/armpits and shook him aggressively while yelling at him on the playground. The child was crying and screaming while the teacher pulled him away by the arm. This is an immediate risk to a child. Type A citation is given. 8

To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs. The staff noticed the child was given the wrong bottle and stopped feeding child immediately, corrected action and notified management. Facility self reported incident to licensing. Parent was notified and staff training was given. Deficiency cleared. This requirement was not met by a child in the infant room was feed the wrong bottle by a staff, the child drank an oz of the wrong milk. This is a potential risk to a child. 8

Prior to employment, a teacher shall meet the requirements of (b)(1) or (b)(2) below: This requirement was not met by: Director stated the staff will not be left alone with children. Other staff did not know the staff was not qualified. The staff is currently attending school to become qualified. Deficiency cleared. An aide was left alone in a classroom with preschool children. This is an immediate risk to children. LPA Owens gave and disccused the LIC 9224 with Director at time of inspection.

Prescription medications shall be administered in accordance with the label directions as prescribed by the child's physician. This requirement is not met 1 Director removed the medication from the medicine box located in the kitchen and will return medication to the parents. She will have a plan to check as evidence by: LPA Owens observed two children prescriptions that were expired: 1- inhaler expired 6/20/21 & 2-Epi-pen expired 8/2021. This is a potential risk to children. 8 medication every month. Deficiency cleared at time of inspection.
