Based on observation, the licensee did not comply with the above by having 2 preschool children in a bathroom without the supervision of a teacher. This poses a potential risk to the health and safety of children in care. 8 Preschool 1 shall have at least two staff members assigned at all times. The facility shall implement methods of communication between classrooms that include, but are not limited to, the use of two-way radios. A memo shall be signed by all staff and documentation of schedules shall be submitted via email by 10/20/2025.
Based on site observations, interview, and record review, the facility did not comply with the above by having teachers supervise more than 12 children at a time without an aide or another teacher present. This poses a potential risk to the health, safety, or personal rights of persons in care. 8 A training memo with meeting minutes detailing the content of the training shall be signed by all preschool staff and submitted to LPA by 8/15/2025. The updated morning schedule shall also be submitted to LPA by 8/15/2025.
