(1) No child(ren) shall be left without the supervision of a teacher at any time...(1) Supervision shall include visual observation. This requirement was not met as evidenced 1 LPA observed that the facility installed locks on the outside classroom doors to prevent children from opening the doors by themselves and new locks were installed on gates. LPA cleared the citation while on site and provided D1 with a letter of clearance. by C1 entering a classroom alone on 04/17/2025; this poses/posed an immediate health, safety or personal rights risk to persons in care. 8
Based on observation, the licensee did not comply with the section cited above when an individual was working in a classroom without a cleared background being associated to the license, which poses an immediate health, safety or personal rights risk to persons in care.
