To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse or other actions of a punitive nature including but not limited to: interference with functions of daily living including eating, sleeping or toileting; or withholding of shelter, clothing, medication or aids to physical functioning. 1 The director stated a training was provided on 3/25/24 regarding appropriate handling of children. Director will provide additional training for Staff 3 by 4/9/24 and send training agenda/notes to LPA Cunningham by 4/15/24 via email to nicolette.cunningham@dss.ca.gov
Cribs shall be free from all loose articles and objects, including blankets and pillows. Based on LPA's observation, one infant was allowed to sleep in a crib with a bottle. This is a potential health and safety risk to children. The licensee she will review safe sleep regulations at the staff meeting on 3/25/24 and send a copy of the agenda with documentation of staff who attended the staff meeting. nicolette.cunningham@dss.ca.gov
7 sleep logs were observed to be missing initials or signatures, signing off the Safe Sleep15-minute checks had been completed. 1 Licensee discussed Safe sleep 15-minute checks with staff on 12/14/2023. Licensee will be having additional training on 2/5/2024 documenting the Safe Sleep 15-minute checks. The training materials and plan will be completed, signed, and employee attendance agenda will serve as proof of correction. The licensee will submit to the Licensing Program Analyst LPA,J.Gifford by 2/16/2024.

Based on interviews, the licensee did not comply with the section cited above, which poses a potential health, safety, or personal rights risk to children in care. 1 The facility immediately put into place a binder with a separate section for each child to ensure diaper changes are made throughout the day. The director implemented a new policy to ensure every staff is trained on operation of every room. Deficiency corrected on this date.

missing documentation for C1, C2, C3, C11, C12, C13 & C14 as based on file review and interviews the facility failed to provide copies of type A violations to Children's representatives. Which poses/posed a potential Health and Safety risk to children in care. 8 indicating they received a copy of the facility report for the type A violations (9099'd pages dated 6/30/21 & 7/2/21) (the rest of the missing documents were already provided or no longer needed)
