Director must submit a plan stating how the facility will ensure supervision when children are in the restrooms. Director will send plan to LPA Pringle by POC date listed. One (1) child was unknowingly left inside the bathroom alone without adult supervision. This poses an immediate risk to the health, safety, and/or personal rights of the children in care. 8 Failure to correct will result in a $100 per day civil penalty until corrected. Repeat violations are $250 per violation and $100 per day until corrected.

Email LPA the agenda for the training and sign in sheet for all staff who attended the training.

Heidy shows on Guardian as "in process". She is required to retake the Live Scan. She left the facility during inspection. Per file review staff HEIDY DELEON VERDUZCO present today did not have working eligible clearances, was not associated to this license or any other licensed facility. Civil penalty of $100 assessed. This poses a potential risk to health safety of children in care. 8 By 2/14/24 Director shall submit a written corrective plan of action ensuring compliance moving forward.
During inspection LPA and Director found a child C1 alone in class bathroom while room children and staff had left for outdoor activity. C1 was left alone without visual supervision for at least 24 minutes. This poses an immediate health, safety or personal rights risk to persons in care. $500 civil penalty assessed.
