(a) The child care center shall be clean, safe, sanitary...at all times...ensure the safety and well-being of children, employees and visitors.(1)...licensee...take measures to keep the center free of flies, other insects, and rodents. This requirement is not met evidenced by: 1 Licensee wil remove dead insects immediately and send photos to LPA via text by 7/2/24. Licensee will have deep cleaning service this Friday 7/5/24 and submit proof of cleaning service receipt. Based on observations and interview, three out of seven rooms had live and dead insects and insect droppings in the areas children use, which poses an immediate health and safety risk to children in care. 8 Licensee will gathered fumigation contract or service receipt from landlord and email to LPA by 8/2/24.

; The facility transporting 1 Licensee states will provide to the department an updated Admission Agreement regarding a new transportation policy for infants in care. Licensee will also submit to the department proof of insurance and photos of the vehicle being used and copies of permission slips from parents. infants with out having permission from the department or documented in the admission agreement and by both of the licensee not forthcoming to the LPA, regarding having a Non employee or fingerprinted adult transport infants in care. This is an immediate risk to the health and safety of children in care.
