Responsibility for Providing Care and Supervision. (a)The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time... Supervision shall include visual observation. Director plans to conduct a name to face training and training transitions with staff. Facility will install additional cameras for the yard and has installed bells for all doors at the facility. Director shall send LPA proof of completed training by staff. This requirement was not met as facility self-reported through an Unusual Incident Report that child C1 was outside alone on the fenced playground for approximately 1-1.5 minutes on 04/21/2025. This is considered an immediate threat to the health and safety of children in care. 8
