(1)The licensee shall take measures to keep the center free of flies, other insects, and rodents. This requirement was not met as evidence by: 1 The facility is to submitt updates in regards to odd odors, mice, mice droppings. The preschool will submit copies of the cleaning check list logs once a month to the Department. Additionally the facility needs to submit a plan to correct the ongoing issue. Based on observation, interview, and record review, the facility has ongoing rodent problems which poses a potential Health and Safety risk to persons in care. 8
