Based on record review, the licensee did not comply with the section cited above when infant files did not contain Blue Cards which poses/posed a potential health, safety or personal rights risk to persons in care.

staff member supervising 5 infants alone for 22 minutues on 03/20/206. This poses a potential health, safety, or 1 Director Fultz will conduct training with all infant staff members regarding ratios and parent pick-up/drop off. Director Fultz will have staff members sign a training reciept and will email this form to LPA Velazquez by 5:00 PM on 04/28/2026. personal rights risk to person(s) in care.

Based on LPA observation, the licensee did not comply with the section cited above when LPA observed 2 prescriptions with no prescription label which poses/posed a potential health, safety or personal rights risk to persons in care.

Infants with an Individual Infant Sleeping Plan that have Section C of the form completed and signed by an authorized representative shall be placed on their back when first laid down to sleep... a. Upon staff observation that the infant is capable of rolling... notify the authorized representative, and obtain the 1 Director Sanchez was able to obtain the authorized representative's signature on the LIC 9227 while LPA was conducting the field visit. Director will schedule maintanence to repair the water outlet as soon as possible. Director will schedule re-testing of the water after the repairs have been completed. LPA will follow-up with the facility once re-testing results have been reported to LPA. The water tested on 06/14/2022 revealed that the facility had elevated levels of lead in one water faucet. This is a violation, that if not corrected, can pose a health and safety risk to the children in care.
