By 11/21/2025, Director stated they will submit a written statement on how facility remain in compliance. Director also stated they will conduct staff meeting on ratios. By COB 10/03/2025, Director will conduct a positive redirection training with S2, the Director has also been observing S2 while in classroom for additional support. Director will submit a statment to LPA upon completion of the training on how S2 and the facility will remain in compliance and have S2 sign When LPA observed footage of S2 grabbing C1 by their ankle and pushing C1's shoulder down forcing the child to lay down on the cot for nap time which poses an potential risk to the health, safety or personal rights of children in care. 8 the form as well. interviews, observations , 1 The center director has since instructed staff to stand in close proximity while providing visual supervision when child are playing on the open slide. Licensee will also conduct an all staff training on playground supervision and submit a summary of the training and a sign in sheet of all staff that attended by 7/12/24. and a review of records which revealed staff was not properly supervising children on the play ground and a child fell from the slide.
