Based on observations, interviews, and record reviews, the licensee did not comply with the section cited above. LPA observed that S1 was supervising 17 children in the playground, and per S1 was left alone on multiple occasions which poses a potential risk to the health, safety, or personal rights of children in care.
Based on observation and documentaion of the Lead Testing Results, the facility has multiple water faucets for lead exposure that exceeds the 5PPB level. This is an potential risk to Health and Safety and/or personal rights risk to persons in care. 8 Director must submit proof of correction to LPA via email, that the lead level is in compliance.
