This poses an immediate threat to the health and safety and personal rights of children in care. 8 Licensee was also informed that an informal meeting with community care licensing may be required.
LPA observation, record reviews and interviews which revealed staff did not ensure supervision was provided to children in care which resulted in a child being injured.
interviews which revealed that a substitute teacher was left to supervise children on the playground, but was not notified of a child's behaviors prior, leading to another child being injured. Since the incident, the facility has met with the parent to support the child in efforts to curb the behaviors. Staff now shadow the child &,engage the child to keep the child busy in efforts to anticipate and gage the child's behavior. The parent will sit in class two days per week. Licensee will submit an update by 2/10/26.
Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above in [count] out of [total count] [(objects) (persons)] [identifiers] which poses/posed a potential health, safety or personal rights risk to persons in care. A REVIEW OF RECORDS REVEALED 3 CHILDREN DO NOT HAVE A PHYSICIAN'S REPORT IN FILE.

the lead testing results received, indicating that two outlets in the facility had results in exceedance of the 5 ppb Action Level. This poses a potential risk to the children in care. 8 The facility will submit the test results once the outlets are retested, and undersands not to use the outlets until the Action Level is determined to be 5 ppb or below.
