Facility submitted new procedures and plan of corrections in place, acknowledge lapses in procedures and stated they have made several changes to ensure 100% visual supervision moving forward. Staff trainings and parent conferences have also been held since incident. Per investigation, staff S1 responsible for picking up school age children including C1 from the elementary school next door returned to facility without C1, leaving child unattended, without 100% visual supervision for atleast 10 minutes from time of pick up to return to center. This posed a potential risk to health/safety of child in care. This deficiency was cleared during today's inspection, Letter of Clearance was provided.
