Based on the facility's UIR and interviews conducted by SS and S1, a child was left unattended inside the preschool classroom. This poses an immediate health and safety risk to children in care. A civil penalty of $500 is being issued for absence of supervision.
Absence of supervision, including, but not limited to, a child left unattended, and supervision of a child by a person under 18 years of age. 1 The center director reported moving forward the center will continue to have alarms to observe the name to face checks. And conduct individual training with the staff to go over "prevention of lost children". Based on Self reported UIR and staff interviews C1 was left alone for approximately 27 mins in the playground. This causes an immediate health and safety risk to children in care. 8 Center director agreed to email over a copy of the "prevention of lost children" training to LPA before the end of the day 12/11/2024.
Based on six staff (S1-S5 & CD) records reviewed at 11:29am which revealed S2 & S4-S5's records did not contain evidence of completion for AB 1207 Mandated Reporter Training. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
Based on statements provided by S1, S2, S5, S6 & S7, P1 and P5 which confirmed they 1 Center Director stated she would have a conversation with the Licensee to come up with a written plan to ensure compliance with the ratio requirements.
