Infants up to 12 months of age shall have a completed Individual Infant Sleeping Plan [LIC 9227 (3/20)], which is incorporated by reference.This requirement was not met as evidence by: 1 Director Lauren Patlan, stated that she will conduct a staff training on 12/23/2025 that address Title 22 regulations. Director will provide a copy of the training agenda and teachers signatures confirming attendance to the department by 12/24/25. Based om interviews, observation, and record review, the facility did not comply with the section cited above in that, the LPAs observe C2 sleeping on their stomach and verified that the LIC 9227 section C was not filled out. Which poses/posed a potential health and safety or personal rights risk to children in care. 8

… 1 Ms. Patlan, the director stated and provided documentation that she has met-one-one and reviewed supervision requirements with the lead teacher of the classroom involved. Additionally, she will address all staff at the next scheduled staff meeting on 8/15/25 or sooner. Based upon incident report received on 7/15/25 , video review, and interview with Director, children C1 and C2 were left unsupervised for approximately 30 seconds after exiting the classroom, which is a potential health, safety and personal rights risk to children in care. 8 Topic addressed: Title 22 supervision regulation and internal supervision. Director states she will provide LPA with signed documentation from both meetings.

… Director stated (and LPA confirmed) that she met-one-one with the lead teacher of the classroom involved, as well as held a staff develop meeting on 2/17/25, in which indoor/outside supervision was reviewed. Director provided LPA with documentation from both meetings. The deficiency has been cleared. Based upon unusual incident report submitted by the facility and interviews with staff, C1 was left unattended outside the classroom and Directors Office, for approximately 2 minutes, which is a potential health, safety and personal rights risk to children in care.
