Based on observations and interviews 1 Director stated that staff has been let go from this facility. Director will conduct training to staff about person rights and will submit proof to LPA. LPA provided a personal right regulation to Director. Based on record review and interview, a teacher lefted a child by their ankles, which poses and immediate health, safety, or personal rights risk to persons in care.
