Based on observation and interview, the licensee did not comply with the section cited above as two (2) children was left unsupervised at the facility in separate incidents, which posed an immediate threat to the health and safety of the child in care. 8 Staff will provide written statements after meeting with the Director that shows their understanding of supervision protocols. Written statements shall be signed and dated by staff. Director will send above documents to LPA via email by 08/26/2026.
Based on observation, interview, and record review, the licensee did not comply with the section cited above as four (4) staff present during the inspection did not have fingerprint clearances associated with the facility, which poses an immediate health, safety, and/or personal rights risk to persons in care. 8 LPA provided Title 22 Section 101170 Criminal Record Clearance regulations to Director for review.
