... 1 Per licensee, staff#4 was given written disciplinary action, staff#4 watched CCL Personal Rights training on line. Also a meeting will be held with staff on 07/03/25 and a copy of staff sign in sheet and topics that will be discussed will be sent to LPA Eivazian. Per four staff interviews, on 06/23/25 child#1 had thrown himself on the floor, where staff#4 picked child#1 up from underarms at which time the bruise would have occurred as child#1 was resisting. This poses an immediate health, safety and personal right to the children in care. 8 Licensee will sent a written plan of correction to LPA Eivazian by 07/03/25.
Based on four staff interviews, on 06/23/25 child#1 was picked up by staff#1 from underarm which cause bruise on child#1 right arm and this incident was not reporter to Licensing Department within 24 hours. This poses a potential health, safety and personal right risk to the children in care. 8 The facility did not report the incident within the required 24 hour time frame.

... 1 Per licensee, Shamalie Goonewardene, she will change her staff schedule effective Monday 10/25/2021 in order to meet Title 22 Regulations Teacher-child Ratio requirements and a written letter will be submit to LPA by plan of correction due date, 10/25/2021 via email. Upon LPA's arrival to the facility at 8:40 a.m. till 9:30 a.m. when facility director, Enereyda Flores arrived, there were 22 children present in the facility with one fully qualified teacher, Melina Avakiankhanejoy and one teacher assistant, Veronica Amador. This is an immediate health and safety risk to the children in care.

Based on LPA Eivazian observations during facility roster review on 10/22/21, LPA observed child#1 was under two years old and will turn two in November 2021 and child#2 was enrolled 05/13/2021 when was an infant and turned two years old in September 2021. This is an immediate health and safety risk to the children in care. 8
