101227(7)(B) Food Services (B) A child shall not be served any food to which the child's record indicates he/she has an allergy. This requirement was not met as evicenced by: Director stated the food is given to the front desk for initial inspection, then ptovided to the classroom. Upon inspection the food items must have an ingredient list. Director will conduct a meeting regarding allergy procedures on 06/11/2024 and submit a signed attendance sheet to LPA by POC date. Based on LPA's interviews and record review, the facility did not follow food allergy care plan. C1 was served pastry with nuts, did not utilize epinephren medication or notify C1's parents of exposure. This is an immediate health, safety or personal rights risk to persons in care. 8
101229(a)(1) Responsibility for Providing Care and Supervision. (a) The licensee shall provide care and supervision... (1) No child(ren) shall be left without the supervision of a teacher at any time... This requirement is not met evidenced by 1 Director states that facility has addressed concern and took appropriate discipline action to correct the issue. Staff will be retraining on suppervision procedures with additional measures being implemented to ensure all children are accounted for during transition. Based on interviews with staff #1, #3, and Director, C1 was left unsupervised in the classroom for approximately 30 minutes. This poses an immediate risk to the safety of children in care. Director will submit proof of staff retraining to LPA by 10/02/2023
Director stated that the staff are reminded every Monday on how to conduct head counts at 3 different points. Director will provide a training with the toddler classroom teachers on effective supervision and headcounts on 08/02/2022. Director stated that she will forward the agenda and what staff attended the During the transition from playground to classroom (elm classroom) Staff #1 and Staff #2 did not condcut the headcount upon entering the classroom with 11 children present. It as observed by staff #3 and #4 that a child was left unattended in a dark classroom for the duration of 10 minutes. Based on observation and interview, Maryam Derakhshami was present at the facility and did not have criminal background clearance.Business Manager stated today was her first day at the facility. This poses an immediate safety risk to children in care.
