The licensee not providing the required LIC9275 External Water Sampler Self-Certification Form, LIC9276 CCC Sampling Checklist Form, and LIC999 Facility Sketch. 1 The licensee agrees to provide copies of the LIC9275 External Water Sampler Self-Certification Form, LIC9276 CCC Sampling Checklist Form, LIC999 Facility Sketch and results of second retest. The plan of correction shall be submitted to CCLD on or before 3/1/2023.
