Based on interview, the licensee did not comply with the section cited above in the facility did not have Lead Test which poses an immediate health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above in 2 out 4 staff did not have fingerprints associated with the facility, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above in 2 out of 4 staff records did not have all required licensing documents, and transcripts which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation, interview, and record review, the licensee did not comply with the section cited above due to Child #2 and #4 not having a Physician's Report or medical examination on file. Both Child #2 and #4 have a copy of a medical appointments on file for dates after the date of enrollment, however, 30 days have passed from the date admission and the date of the medical appointment on file and still no actual medical examination or Physician's Report is on file which poses a potential health, safety or personal rights risk to persons in care.
