it was determined that Staff #1 engaged 1 Licensee stated she will write a written statement with the plan of correction by the next business day 08/21/25 reaching out to KIngs County Behavioral for training purposes. in inappropriate physical contact toward two children (child #1 and #2) resulting in minor injuries. These incidents pose an immediate risk to the health, safety, or personal rights to children in care in that inapproprioate physical contact was demonstrated and used by staff #1 to child #1 and child #2.
Staff #1 yelled at Child #2 while attempting to enforce a time-out, repeatedly raising her voice and demanding that the child sit down. 1 Licensee stated she will write a written statement with the plan of correction by the next business day 08/21/25 reaching out to KIngs County Behavioral for training purposes. Staff #1 was also observed yelling at children on multiple occasions which poses an immediate risk to the health, safety, or personal rights to children in care.
Staff #1 and Staff #2 observed Child #1 1 Licensee stated she will write a written statement with the plan of correction by the next business day 08/21/25 reaching out to KIngs County Behavioral for training purposes. and Child #2 rough housing in the classroom. Despite being instructed to stop, the children continued to engage in physical fighting in which staff neglected to intervene which resulted in child #1 sustaining minor physical injuries in the facial area. This is an immediate safety risk to children in care.
It is determined that chairs are used not as tools for regulation or redirection, but rather to threaten, demean, label and physically punish children for 1 Licensee stated she will write a written statement with the plan of correction by the next business day 08/21/25 reaching out to KIngs County Behavioral for training purposes. exhibiting maladaptive behaviors. This poses an immediate risk to the health, safety and/or personal rights of children in care in that the use of “time-out” is to be used appropriately in accordance to the dignity of children in care.
results from the water lead sampling test showing an ALE 1 Owner stated the facility will notify parents and/or authorized representatives of the lead exceedances and the corrective action that has already taken place. Owner also said water source (H) will permanently removed because they are now using an Igloo filled with water every day as the water source for children. of 11 ppb in water source (H) and this poses a potential risk to the health, safey, and/or personal rights of children in care. A copy of the letter informing parents of the exceedance will be submitted to CCL by 2/28/23.
Based on LPA's review of staff records, the licensee did not comply with the section cited above because staff had not taken the required mandated reporter training, which poses/posed a potential health, safety or personal rights risk to persons in care.
