Based on LPAs interviews and observations, C1 and C2 climbed 2' high x 4' wide bookshelves repeatedly and incurred 2-3 injuries which poses a potential risk to the health, safety, and personal rights of persons in care. ensure the furniture and environment are safe. AD1 will provide a copy of the inspection report from 5/15 to 6/12.

(a)The licensee shall ensure that each child is accorded the following personal rights: (1) To be accorded dignity in his/her personal relationships with staff and other persons. This was not met at evidenced by: 1 D1 stated she will review the facility’s Personal Rights policy requirements to ensure understanding of appropriate behavior and guidance techniques and will implement a training for all facility staff. Documentation of the completed training will be provided to jennifer.patel@dss.ca.gov D1 acknowledged she yells and interviews with staff corrobrorated that staff yell which poses a potential risk to the health, safety, and personal rights of persons in care.

Personal Rights...: To be free from.. ., infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse or other actions of a punitive nature... this was not met as evidence by... 1 Area supervisor Vanessa Stevenson reported to ensure this doesn't occur staff training will be provided. LPA provided the website "California early childhood online" as a resource for training videos. based on evidence received and interviews conducted there was at least one occasion were staff yelled at children. This poses a potential health and safety risk to children in care. This is a repeat citation resulting in civil penalty $250. ***This is an amended report*** 8 Facility representative requested time to ensure all staff and new hires are able to participate in the training. Facility representative agreed to send staff training minutes and sign in sheet to LPA via email.

Buildings and Grounds (a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. This was not met as evidence by... 1 Regional Supervisor taped the holes in the canopy to prevent more wasps from entering, and took down the wasp nests that were visible on the canopy. The metal mesh fence was zip tied to prevent children from pushing their hands/ arms though... .based on interviews conducted and observations made the center did not ensure the outdoor play area was free of wasps and the metal mesh fence was repaired. This causes a potential health and safety risks to children in care. 8 Regional Supervisor reported the metal mesh fence will be wielded for a more permanent solution. And the tubes of the canopy have been taped, no wasps will be able to make nests there. The center also has an pest control company that sprays around the building once a month.

This was not met as evidence by... 1 Regional director agreed to conduct a training with all staff present. Center director reported previous staff no longer work at the center who may have been involved with the violation but to ensure it doesn't occur in the future staff training will be completed. Based on interviews one or more staff members used loud, stern voices at the whole class and/or at an individual child in an aggressive manner. This poses a potential health and safety risk to children in care. 8 Regional director agreed to continue and model the behavior for the staff and submit training agenda and staff who were present and submit to LPA Hernandez Torres via email, mail or fax on or before 01/12/2024.
