Based on LPAs observation, the licensee did not comply with the section cited above in three out of four crib mattresses had tears in the vinyl coverings. Therefore the foam was exposed and the mattress was not in safe condition which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on LPAs observation, the licensee did not comply with the section cited above when the bathroom floor had a tear in the flooring material which was approximatley 5 feet long by two inches wide. The tear had been covered with duct tape. Therefore the bathroom was not maintained in safe and sanitary operating condition, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on LPAs observation, the licensee did not comply with the section cited above because the first aid kit was missing a current edition of a first-aid manual (1999 was on site), antiseptic solution and adhesive tape, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on LPAs observation, staff interview and record review, the licensee did not comply with the section cited above when the facility failed to document the November 2021 disaster drill, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on LPAs observation, the licensee did not comply with the section cited above in one of one changing pad. The infant changing table pad had approximately six areas where tape was applied over rips/tears in the vinyl or plastic covering. Based on LPAs observations the equipment was not in good repair, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above in seven out of ten infant files. Per record review the needs and service plans were not updated every quarter as required, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on LPAs record review, the licensee did not comply with the section cited above in Ten out of ten infant files reviewed did not contain documentation of the required 15-minute check. The time of each check was missing as well as the initials of the person who conducted each 15 minute check, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on LPAs record review, the licensee did not comply with the section cited above in four of four infant files for children under 12 months of age did not have an individual infant safe sleep plan (LIC 9227), which poses/posed a potential health, safety or personal rights risk to persons in care.
