(E) If an infant falls asleep before being placed in a crib, staff shall move the infant to a crib as soon as possible. Licensee will have meeting and or individual training to have staff review the safe sleep regulations. Licensee will submit a document signed by all infant staff members that includes their commitment to following safe sleep regulations. Licensee will submit plan to LPA by 12/24/25. Based on observation and interview LPA witnessed infant in care sleeping in S1's arms for an extended period of time. Based on observation and interview, the licensee did not comply with the section cited above for one infant was observed sleeping in staff arms which poses an immediate health, safety, or personal rights risk to children in care. Licensee will submit training documents to LPA by 1/3/25. Submit documents to LPA at tammy.dutra@dss.ca.gov.

This requirement was not met as evidence by: 1 Facility representative will train all employees who work with infants on how to properly use high chairs at the facility. An acknowledgement signed by all infant staff and be emailed to LPA at: emily.curiel@dss.ca.gov by 1/23/26. Responsibility for Providing Care and Supervision for Infants (B) Staff shall physically check on sleeping infant(s) every 15 minutes and document the following:1.Labored breathing. 2. Signs of distress, which includes but is not limited to flushed skin color, increase in body temperature and restlessness... 1 Licensee agrees to train all infant staff members to document 15 min sleep checks for all infants in care 24 months and under. Licensee agrees to send in three days worth of 15 min checks for 3 infants in care by 1/3/25. Send documents to LPA at tammy.dutra@dss.ca.gov. (C)Documentation shall be maintained in the infant’s file and be available to the Department for review. Based on observation and record review it was determined that staff are not recording and maintaining safe sleep logs for infants in care.
