Infant Care Personal Services §101428(b)(2)

Based on observation, interviews, and record review, Staff 2 and Staff 3 did not ensure infant’s diapers were changed as needed to ensure the infants remain clean and dry at all times. Statements from Infant 1 and 2 Parents confirms children were picked-up in heavily soiled diapers that soaked through their clothing. The daily log showed that Infant 1, 2, 3, and 5 went over 3 hours on 1/4/2023, 1/5/2023, and 1/9/2023 without being changed which poses a potential health, safety, or personal rights risk to persons in care.
