After careful review of the transcripts by Community Care Licensing, it was determined that Staff 1 does not have the required units to be a teacher at the facility. Staff 1 is missing 3 units of Curriculum and 3 units of Child, Family, and Community. This poses a potential health, safety, and personal rights to children in care. 8 Request Denied: Appeal Not Submitted Timely

The Director will conduct an in-service training with all staff on the importance of ensuring compliance within this section. Copies of the training along with signatures of all attendees will be submitted to CCL by the

LPA observed C#1 in the front play yard of the facility alone, unsupervised and unable to enter into the facility. This poses an immediate risk to the health and safety of children in care

On 04/13/2021, LPA Martinez observed 2 school-age 1 The licensee shall immediately ensure that at no time will any school age children be allowed entry into the preschool for any reason. The school age children must not use the preschool restroom, nor the preschool playground. The licensee shall submit a written declaration to acknowledge children commingled with the preschool children in the preschool classroom. This is a Type A citation and poses an immediate health and safety risk to children in care. 8 understanding that commingling of school age children on the preschool premises is prohibited and will cease immediately. The declaration is to be mailed/emailed to the Department no later than close of business on 04/14/2021.
