Supervision shall include visual observation. This requirement was not met as evidence by: 1 Center is to conduct staff training regarding Supervision to children in care and classroom management. Proof of training has been submitted to the LPA during today's visit . Center will be having 2 teachers in the class supervising and doing face to child count and documenting the count and truing it at the end of the night. Center has director and assistant director monitoring the cameras. Staff will also go around the classroom and restrooms doing checks. Each teacher will be given 1 on 1 training. Based on LPA's observations and interviews the licensee did not comply with the section cited above as a child was left unattended for more than 5 minutes which posed a potential health, safety or personal rights risk to persons in care. 8

101212 Reporting Requirements(d)(1) Upon the occurrence...of the events specified below, a report shall be made to the Department...Events reported shall include the following: Any unusual incident or child absence that threatens the physical or emotional health or safety of any child. This requirement was not met as evidence by: 1 Center is to report new director information to the Department. Directors are to watch the Child Care Reporting Requirements video located at ccld.childcarevideos.org/child-care-center-operators/. The center is to submit proof of director qualifications for all three staff who hold a designation of facility responsibility. Based on observation, interview and record review, the licensee did not report incidents which poses potential Health and/or, Safety and, Personal Rights risk to persons in care. 8
Admission Agreement (f) The licensee shall comply with all terms and conditions set forth in the admission agreement. This requirement was not met by evidence by: 1 Licensee is to submit a written plan indicating what steps will be taken in order to prevent parents from bringing treats that may contain nuts. Licensee is also to update the March Newletter to contain "nut-free" wording and submit proof to LPA by 3/21/23 Based on observation, interviews conducted, and record review, Licensee did not comply with the deficiency cited above as assistant director states a non-labled treat was allowed in a classroom which poses a potential risk to the health, safety and or personal rights of children in care. 8
Food Services(7)Modified diets prescribed by a child's physician...shall be provided. (B) A child shall not be served any food to which the child's record indicates he/she has an allergy. This requirement was not met by evidence by: 1 Licensee is to submit a written plan on how they will approach parents who bring treats that contain peanuts. The plan shall be submitted to the Department by 3/21/2023. Based on observation, interviews conducted, and record review, Licensee did not comply with the deficiency cited above as assistant director states the goddie bags were not checked prior to this to ensure all treats were nut free which poses a potential risk to the health, safety and or personal rights of children in care. 8
