101229 Responsibility for Providing Care and Supervision (a)(1) No child(ren) shall be left without the supervision of a teacher at any time.... Supervision shall include visual observation. This requirement is not met as evidence by: Director provided a written statement regarding that she counseled each staff member regarding supervision and moving forward a permanent staff member will always be present with a substitute teacher from an outside resource during nap time and during the day to ensure supervision at all times Based on LPA’s interviews, 2 out of 2 staff stated the incident regarding S1 walking outside of toddler classroom while toddlers were napping for approximately 1 to 2 minutes. This action poses a risk to the health and safety to children in care. 8

Baesed on observation on 09/05/2023, a window screen was partially torn open from.. 1 During visit on 10/13/2023, LPA observed window screen in good repair. the bottom right corner. Window is located above foam climbing block equipment that is used by infants. This poses a potential health and safety risk to children in care.

Based on interview with staff, S1 slid C1 forward to move child back. S1 admitted to have handled it by getting up instead of pulling C1 towards S1. This poses a potentail health and safety risk to children in care.
