a) The licensee shall ensure that each child is accorded the following personal rights: (2) To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs. 1 Child care center(facility) will remove direcor's dog, when children in care are present at facility. Based on LPA interviews, and documents collected during the investigation, the above mentioned regulation was not met. LPA discovered, directors dog was present during operational hours. Directors dog nibbled on daycare children. 8
Staff#2 did not have a current active AB1207 during LPA inspection/record review on 02/5/2025. LPA observed Staff#2 providing care and supervision to chidlren on 2/5/2025.
Based on interviews and record review which revealed a high chair was used in order to restrain a child with challenging behaviors. This poses an immediate risk to the Health and Safety of childrenin care.
Based on interviews and record review which revealed a child was left alone outside in the sandbox while children and staff transitioned inside. In addition, there was a child who injured their head while in care but neither individual witnessed the event. Neither unusual incident was reported to CCLD. This poses an immediate risk to the Health and Safety of children in care.
Based on interviews and record review which revealed Director did not report the incidents in a timely manner which occurred in June - October 2021.
