Per Lead Teacher, the facility will remain within their fire clearance limit and facility licnese capacity. Will provide a traning plan by the plan or correction due date 08/25/2025. The licensee did not comply with the section cited above in that there were 42 children in care that exceeded the facility fire clearance which poses an immediate health, safety, or personal rights risk to persons in care.

Per Lead Teacher, facility going foward will allow the Department to inspect any documents in the facility. The licensee did not comply with the section cited above in that Director on 06/18/25 did not allow inspection of documents at the facility by Departments representatives which poses a potential health, safety, or personal rights risk to persons in care.

Based on observation, interview, record review, the licensee did not comply with the section cited above in that LPM and LPA observed 42 children present the addtional children were from facility # 197493615 which poses an immediate health, safety or personal rights risk to persons in care.

Personnel Records(d) All personnel records shall be maintained at the child care center and shall be available to the licensing agency for review. This requirement is not met as evidence by: 1 Director stated that the records will be updated and will email LPA a copy of the documents by the plan of correction due date of 06/25/2025. Sign In and Sign Out, (d) The sign-in and sign-out sheets with the signatures required ... shall be kept for one month and shall be available at the center for review by the Department.
