101229 Responsibility for Providing Care and Supervision(a)The licensee shall provide care and supervision...to meet the children's needs.(1)No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This requirement was not met as evidence by: 1 The center has placed a wooden board to fill the gap at the bottom of the gate. Based on interview and observations, the licensee did not have supervision of all children in care which poses an immediate Health and/or, Safety and/or, Personal Rights risk to persons in care. 8
Facility has taken the following steps: Shutoff and labeled the two (2) outlets, and will notify parental notification of the water testing results, and will be removing the outlets. Facility has until 10/23/2022 to correct the deficiencies. Facility conducted water lead testing and had two (2) outlets exceeded the action level exceedance per regulations. This poses a potential risk to the health and safety of the children in care
