Based on LPA’s interviews, 6 out of 8 staff disclosed that the facility did not conduct disaster drills in December 2023 or in the last 3 years. 5 out of 8 staff stated they do not feel sufficiently trained especially when it came to the facility’s emergency plan.

Based on LPA’s interviews, 5 out of 8 staff stated the previous director did not report to parents and to Licensing of a Hand Foot Mouth (HFM) outbreak. LPA reviewed the file and confirmed facility did not notify the Dept. about the HFM outbreak from last year. 8 BrightWheel informing parents of the facility's policies and procedures for Health and Safety. Director will email LPA screenshots of the BrightWheel and the health guidance attachment,

Buildings and grounds §101238(a)
interview with 3 staff who indicated a cabinet fell in Mandarin Toddler classroom on 10/29/21 before the 1 Director was pro-active and immediately hired a contractor to inspect all cabinets which were removed for inspection. After Townhall meeting it was determined cabinets would be anchored and stacked. During today's visit LPA observed cabinets in rooms and most were stacked and anchored, others were set side by side and anchored.; cleared during visit facility was open.This poses a potential danger to children in care.
