Staff #3 was unable to provide proof of immunization records and did not have a copy inside the file which poses/posed a potential health, safety or personal rights risk to persons in care.

Director listed on Licensing Information System (LIS) is no longer an employee at the preschool and the Director in charge has not submitted a Directors packet to Community Care Licensing Division (CCLD) which poses/posed a potential health, safety or personal rights risk to persons in care.
