(a) The licensee shall ensure that each child is accorded the following personal rights:(1) To be accorded dignity in his/her personal relationships with staff and other persons. This requirement has not been met as evidence by: 1 Principal follow up on further actions taken by facility. Based on interviews and record reviews, Staff #1 (S1) handled C1 in a rough manner, placing child on cot. S1's behavior posed a potential personal rights and health safety risk to the children in care. 8

This requirement has not been met as evidence by: 1 Principal agrees to speak to staff regarding Personal Rights. Principal will send a summary of what was duscussed and a follow up on further actions taken by facility. Based on interviews and record reviews, children were present in the classroom at the time Staff #1 (S1) had a loud verbal outburst. S1’s behavior occurred in front of the children and created a potentially frightening and unsafe environment. This posed a potential personal rights risk to the children in care. 8
(a) The licensee shall provide care and supervision...(1)No child(ren) shall be left without the supervision of a teacher at any time...Supervision shall include visual observation. 1 Prinicipal agrees to submit updated plan of action for S1. Principal completed staff training on March 3,2026. Staff meeting topics included: Title 22 Supervision, Sign-in/Sign-out requirements and Staffing Responsibilities. Based on interview and record review, C1 was walked out of the facility door by which was opened by S1. S1 walked away and closed door leaving C1 without visual supervision by a facility staff, which posed an immediate Health and Safety risk to children in care. 8

Based on observation, the licensee did not comply with the section cited above in LPAs observed Ajax, Clorox wipes, hand sanitizers, Clorox bleach spray, Lysol Disinfectant accessible to children in Room 1 and Room 2 which poses an immediate health, safety or personal rights risk to persons in care.
