To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse or other actions of a punitive nature including but not limited to: interference with functions of daily living including eating, sleeping or toileting; or withholding of shelter, clothing, medication or aids to physical functioning. 1 Per Director She will recieve documentation on the roles and responsibilites of the mid level, manager and documentation parent has canceled services with mid level manager.
(a) The licensee shall provide care and supervision as necessary to meet the children's needs. This requirement was not met evidenced by: 1 Per Director, break schedules were changed so that there are 2 teachers at all times. She implemented active supervision training in the annual staff development. Copy of new break schedule and staff development training was provided to LPA during visit. Based on observations made, one teacher was alone with 15 children ages 2-5 for more than 10 minutes and was not able to actively supervise the children and redirect their fighting, which poses/posed a health and safety risk to children in care. 8 Per Director, facility hired a behavioral specialist who came in November and trained staff on challenging behaviors. Copy of training material was provided to LPA via email during todays visit.
(1) The licensee shall provide a variety of age-appropriate equipment, toys and materials in good condition and in sufficient quantity to allow children present to fully participate in planned activities. This requirement was not met evidenced by: 1 Per Director, she spoke with staff effectively immediately all metarials need to be taken into the play ground and kept there Monday through Friday. LPA's observed all materials in the storage brought out and set up in the play ground during visit. Based on observations there were not enough toys and materials for children to utilize causing fights between children, which poses a health and safety risk to children in care. 8
(a) The licensee shall provide care and supervision as necessary to meet the children's needs. (1)No child(ren) shall be left without the supervision of a teacher at any time... This requirement was not met evidenced by: 1 Per director, teachers will be counting children before leaving for breaks and tranisitons. They will modify breaks so they are not during transitions. Director will email updated break schedule to LPA by 5/17/24. Based on incident report, interviews, and observations one child wandered away from facility and was brought back by police officer. 8 Director will add door bells and alarms to the preschool doors and will add a baby gate restricting access to staff door and blind spot by 5/24/24 and send photos of installed devices via email by 5/24/24.
(c) To be a fully qualified teacher...(1) Twelve...units...child development completed...include courses that cover... child growth; child, family and community... and program/curriculum. This requirement was not met evidenced by: 1 Per Director, she is not going to leave Teacher #1 alone with children. Director will send updated personnal report to LPA by 5/20/24. Per Director, she will register and pay for the teacher to complete the required course. Based on record review, one out of two teachers did not have the qualified units to be a fully qualified teacher. Teacher #1 was left alone with 12 children while Teacher #2 took a 10 minute break. 8

SS Cordova propsed possibly closing the Center during construction, negotiating in the contract that contractors work after hours and/or weekend. The kitchen has been undergoing remodeling since June 25. The area is not blocked off and LPA observed that the passageway for the infants to reach their playground had construction materials in it, *This poses a potential health, safety, and/or personal rights risk to the children in care.*
Estimates have already been sought. Once estimates are obtained, they will be provided to LPA Bell. LPA observed dilapidated wood, three holes in the flooring, a rain gutter which needs to be repaired, a rug in front of a classroom door which was tattered and a missing handle on the cabinet underneath a sink in Room 4. *This poses a potential health, safety, and/or personal rights risk to the children in care.*
