Based on a review of records, the center did not comply with the above-cited regulation. Its current practices do not align with the written policies that remain in effect. It was found that certain procedures described in its written materials are no longer being implemented, and the written policies have not been agreed upon by an authorized representative to reflect these changes. Not operating in accordance with the approved Plan of Operation and written policies poses a potential health and safety risk to children in care.

This poses a potential health, safety, or personal rights risk to persons in care. 8 In addition, copies of the External Water Sampler Self-Certification Form (LIC9275), Child Care Center Sampling Checklist (LIC9276), and Facility Sketch (LIC999) need to be submitted to LPA by 8/31/2023.
