Based on observation and record review, the licensee did not comply with the section cited above when an infant was sleeping on her/his stomach when Infant Sleep Plan was not signed by parents declaring that the infant rolls on his/her own, which poses an immediate health, safety or personal rights risk to persons in care.

Based on LPA 07/30/2024 observation and record review, the licensee did not comply with the section cited above when an infant was observed to be sleeping on his/her stomach, and the Individual Sleep Plan did not provide parent proof of infant's ability to roll, this poses/posed a potential health, safety or personal rights risk to persons in care.

a child being served the incorrect bottle on 07/11/2024. 1 Director Gill (D1) created a face-to-name bottle check procedure. The staff were trained on the new procedure on 07/15/2024. D1 supervised that the staff are implementing the new procedure on 07/17/2024. D1 provided LPA with a Face-to-Bottle Observation checklist. This poses/posed a potential health, safety, or personal rights risk to persons in-care. 8 LPA cleared the citation at the time of the field visit.

Based on record review of three staff files did not have current mandated reporter training certificates. This poses a potential health and safety risk to children in care.
