a self-reported incident that occurred on 12/2/2025. Child 1 was left unattended in a classroom for approximately 1 minute before they were found by staff. This posed an immediate risk to the health and safety of children in care.
. Responsibility for Providing Care and Supervision. (1)No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This requirement was not met as evidenced 1 Staff was placed on aministrative leave in order for the facility to conduct an investigation. Staff were retrained on supervision. Staff now follow new procedures when children have to use the bathroom while using the playground. by the unusual incident report, which was reported to LPA on 10/03/2023. Child #1 was left in the bathroom on the playground with the door closed for approximately 1 minute. The child was not injured. This is a potential risk to the health and safety of children in care. 8
