Based on observation, interview and record reviews, Staff 2 does not have infant/toddler units and is not enrolled in any infant/toddler class, and Staff 5 is an aide with no ECE units, both staff were at the Pink room #116 (infant room). This poses a potential risk to health and safety of children in care.
Director stated that moving forward they will call every time there is an unusual incident. A document about the plan of action will be provided to LPA by due date of 10/24/25. Based by staff interview. Facility failed to report to licensing authority about the police investigation that took place on August 8, 2024. Director stated, Police and Social worker stated not to report to Licensing Department. This pose a potential risk to the health and safety of the child in care.
Based on observation and interview, the licensee did not comply with the section cited above in 1 out of 1 item. LPA observed a changing table was set in the middle of Pink classroom away from the washing sink, Director confirmed when interviewed, which poses a potential health, safety or personal rights risk to persons in care.
