101229 Responsibility for Providing Care and Supervision (a) The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This requirement was not met by: 1 Director stated Teachers are to use the face to name on ProCare. Physical Tracking, Teachers are to use sign in sign out sheets near the doors of the classroom. A headcount sheet was also added to all areas where students occupay. Based on LPA observation and directors statement on the unusual incident (UIR) submitted to the office, it was revealed that a child was left alone on 9/19/2024, without any adult supervision on the playground for five minutes which poses an immediate health, safety or personal rights risk to persons in care. 8
