Director will ensure all families receive written notification of basic rate change within the days requiered, 30 days. This requirement is not met as evidence by: notification was provided on 9/3/2025 for a due date of 10/1/2025 for new tuition rate. 8

(1) Events reported shall include the following:(C) Any unusual incident or child absence that threatens the physical or emotional health or safety of any child. 101223 (a)(2) Personal Rights (a) The licensee shall ensure that each child is accorded the following personal rights: (2) To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs. Director agrees to ensure all hazardous items such as tools and equipment will be made inaccessible to the children in care. Director will submit a plan and/or procedures to LPA via email. This requirement is not met as evidence by, based on investigative interviews it was disclosed that facility is not providing comfortable accommodation for children by having hazardous items such as ladders left in the outdoor yard and tools left in the garden area accessible to the children in care, which poses an immediate health and safety risks to persons in care. 8

LPA provided director with a copy of regulation for review. Statement will be emailed to LPA. (a) The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. Director and owner agree to provide a staffing plan to LPA detailing staffing plans for the center. This requirement is not met as evidence by, per LPA Laureano's observation,Staff 1 was left supervising 14 chidlren between 12:08-12:14, providing inadequate supervising to the children in care which poses an immediate health and safety risks to persons in care.

(e) All individuals subject to a criminal record review pursuant to Health and Safety Code Section 1596.871 shall prior to working, residing or volunteering in a licensed facility: (1) Obtain a California clearance or a criminal record exemption as required by the Department or Director and owner agree to have Staff #4 cleared and associated to facility before Staff 4 is allowed back in the facility. Facility will notify LPA via email when it has been completed. This requierment is not met as evidence by, per staf file review and Guardian review, Staff #4 does not have a current clearance associated to the facility. 8
