Based on interviews conducted, documentation reviewed, and observations made, it was revealed that the facility failed to provide adequate supervision for the two subject children, due to them being to leave the facility. 8 Head of school agrees to submit documentation of the trainings, including the agenda and staff sign-in sheets once completed. Head of school also agrees to submit a plan of action moving forward to prevent this type of incident from happening again.
Based on record review, the licensee did not comply with the section cited above in count of 3 persons, in which poses a potential health, safety or personal rights risk to persons in care. LPA observed that Mandated Reporter Training Certificates were expired for S3, S5, and S6.
Based on record review, the licensee did not comply with the section cited above in which which poses a potential health or safety to persons in care. LPA observed that S8 was missing documentation of proof of immunity to Tdap.
Based on record review, the licensee did not comply with the section cited above in which poses a potential health or safety or risk to persons in care. Upon record review, LPA observed that a LIC 503 was not on file for S1 and S4.
Based on LPA observation and record review, S1 was working in the preschool classroom without a completed fingerprint clearance which poses an immediate health and safety risk to persons in care. 8 Facility representatives stated S1 will not return to the facility until a criminal record clearance is obtained.
Based on LPA record review, the licensee did not comply with the section cited above in the director Kim Montague was unable to provide proof of the Health Safety Nutrition and Lead training, which poses a potential health, safety or personal rights risk to persons in care.
California Lead Action Level at Child Care Centers A result with values of 5.5 ppb or greater shall be deemed an Action Level Exceedance. This requirement is not met as evidence by: 1 Facility will implement corrective action pursuant to section CCR 101704 for immediate cessation of outlets testing with action level exceedance until it is replaced and retested pursuant to section CCR 101705 and returns a result at or below the Acton level. Based on records review of required lead testing, the facility had lead values of 5.5 or above on water outlets : HH (tested at 20 ppbs) and X ( tested at 6 ppbs) This is a potential health and safety risk to persons in care. Facility will notify CDSS with completion within 30 days.
