Licensee agrees to map out a staff member scheduling sheet that highlights what teachers go where, according to their ratio. Licensee also agrees to establish a backup plan/policy on what is done incase any staff member does not show up to work. LPA was at the facility on separate matters. During the intial census, LPA observed a total of 11 children in care in the preschool classroom. LPA then observed 2 more children that arrived, totaling the ratio to 1 teacher with 13 children. 8 Licensee agrees to complete and submit plan of correction to LPA no later than close of business on 10/22/2025.
Based on record review, the licensee did not comply with the section cited above in by admission the director Lalita Bakshi, stated that the facility has been tested for lead, which poses a potential health, safety or personal rights risk to persons in care.
