Based on observation, interview conducted, and records reviewed, the director did not comply with the section cited above as director did not ensure mandated reporter training was completed by the expiration date for one out of five staff present which poses a potential safety risk to children in care.

Based on observation, interview conducted, and records reviewed, the director did not comply with the section cited above as it was revealed S1 has been working at the facility since 02/18/2025 without obtaining a criminal record clearance or exemption which poses an immediate safety risk to children in care. 8 Director understands S1 cannot return to the facility until a criminal record clearance or exemption is obtained. A civil penalty of $500 is being assessed and provided.

Based on record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. LPA observed the facility did not test for lead by 1/1/23.

101700.3 (b)(2): Licensees shall maintain a lead value at ... 5 ppb in all outlets subject to the testing requirements of these Written Directives... This requirement was not met as evidence by: 1 Facility will implement corrective action pursuant to section CCR 101704 for immediate cessation of outlets testing with action level exceedance until it is replaced and retested pursuant to section CCR 101705 and returns a result at or below the Acton level. Based on records review of required lead testing, the facility had lead values of 5.5 or above on water outlet: K (6.7 ppb); This is a potential health and safety risk to persons in care. 8 Facility will notify CDSS with completion within 30 days by 5/15/23.
