interview and surveillance that showed a 1 Director, Serena, stated she would provide training to staff regarding handeling children and personal rights. Serena, agreed to send LPA a copy of the training sign in sheet to clear plan of correction by May 3, 2024. staff member jerked a standing child by the arm and the child's knees buckled to the cot for nap time which is an immediate health and safety risk to the children.
Based on observation, the licensee did not comply with the section cited above in that there were 3 awake preschool children and 13 napping preschool children being supervised by the director at the time that LPA Lea Habtom arrived which poses an immediate health, safety or personal rights risk to persons in care.
Based on record review, the licensee did not comply with the section cited above in that a mandated reporter training was expired which poses/posed a potential health, safety or personal rights risk to persons in care.
Personal Rights: The licensee shall ensure that each child is accorded the following personal rights: To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse or other actions of a punitive nature...This requirement is not met as LIcensee shall conduct a staff training on children's personal rights. To correct the deficiency, licensee shall submit a summary of the training as well as employee signatures of who attended the training. Proof of correction can be mailed to the office or emailed to LPA @amanda.blesi@dss.ca.gov. evidenced: Evidence gathered revealed staff #1 grabbed a child by the arm and yanked the child to the ground by pulling on the child’s arm in a rough and aggressive manner. 8
