center failed to report an unusual incident that occured on 02-05-2024 stating day care children being locked out of the facility w/ Staff #1 8 a written report containing the information required shall be submitted to the Department within seven days following the occurrence of such event.

child #1-3 disclosed that on different occasions when the children were not following directions; The facility will undergo training on how to work with children that do not follow directions to ensure this type of incident does not reoccur. Staff shall participate in the training no later than Feb 02/02/2024. Staff #1 would grab the children in a rough manner causing pain because they were not following directions. This is a Type B citation that if not corrected could pose a potential health and safety risk to the children in care. The training will help staff develop effective strategies to handle and engage with children who may struggle to follow directions.
